Complex Psychiatric Case Review (Records-Based)

  • A Complex Psychiatric Case Review is a consultative service for patients or clinicians who need expert evaluation of a challenging psychiatric case based entirely on existing documentation, without a live appointment with the patient. This service is particularly useful when:

    • The patient has a complex medication history or multiple failed treatment trials

    • There is diagnostic uncertainty or overlapping psychiatric conditions

    • Clinicians are seeking guidance on medication sequencing, augmentation strategies, or polypharmacy management

    • Additional expert insight is desired before initiating advanced treatments such as TMS, ketamine, or ECT

    What the Review Includes:

    1. Thorough review of records, including:

      • Psychiatric evaluations and progress notes

      • Medication history and treatment response

      • Laboratory and relevant medical documentation

    2. Identification of treatment gaps or potential alternative strategies

    3. Evidence-based medication recommendations tailored to the patient’s clinical history

    4. Written summary report, which can be shared with the treating clinician or care team

    5. Optional follow-up consultation call for clarifying recommendations

    Important Notes:

    • This is a records-based consult only; no direct prescribing occurs

    • Emergency or crisis management is not included

    • Designed to support and inform ongoing care, not replace it
